Start with a clear nonconformity record
DÖF is a commonly used Turkish abbreviation for corrective and preventive action. This guide focuses on corrective action following an identified nonconformity. What was expected, what was found, and which record supports the finding? Answering these questions helps the team address the same problem. ISO 9001:2015 also addresses prevention through risk-based thinking; assess impact and recurrence risk instead of applying the same lengthy form to every issue.
Separate correction from action on the root cause
Imagine a shipment carrying an incorrect label. Isolating the goods and replacing their labels addresses the immediate problem. Preventing recurrence requires investigating why the wrong label was selected. Access to an old template, an unclear approval step or a missing check are possibilities to investigate using evidence. This scenario illustrates the process; it is not a customer case study.
Connect the root cause to evidence
Writing ‘human error’ alone may not produce a useful improvement plan. Review the relevant records, instruction and working conditions together. Record how the proposed cause explains the finding. Training is not an automatic solution to every incident; if access or controls caused the problem, action may need to change that arrangement.
Define an owner, a date and an output
Assign an owner and target date to each action. Replace broad statements such as ‘improve the process’ with a specific change and evidence of completion. In our example, the team could control access to the effective label template and review the shipment check. If work is late, make the reason and revised plan visible; moving a date forward does not complete the action.
Assess completion and effectiveness separately
Completing a task does not by itself show that recurrence risk has decreased. Plan an effectiveness check with a timeframe, sample and criterion suited to the action. For example, examine selected later shipments to check that the correct template was used. If the outcome is insufficient, reassess the cause and plan. Connect the closure decision to the evidence reviewed and the person making that decision.
Trace one record in a QMS demo
Use a representative record to assess SEZHEL QMS’s nonconformity, CAPA, root cause, action, effectiveness and closure workflow. The product supports traceable responsibilities and evidence; it does not replace the company’s quality decisions. Before a pilot, agree how your process should handle the following situations.
- Can you find the source of the nonconformity and the related finding?
- Can immediate correction and corrective action be tracked separately?
- Are overdue actions and their owners visible?
- Can the effectiveness evidence and outcome be recorded?
- Can you trace reassessment after an insufficient outcome and the closure decision?
